Default win7 screensaver / screen off / hiberate timer problem

my wife's newish (last november) computer has win7 x64
we have the screensaver set @ 5 minutes
screen off @ 10 minutes
hibernate (when on battery) @ 60 minutes

though, randomly it will sometime sit there forever (30+ minutes) without doing anything. and there is no rhyme or reason to it. sometimes it will fail to engage the screensaver with the same webpages open in firefox, same 3 excel documents open, etc. then other times with the exact same things it will start the screensavers, blank the screen, etc.

i have tested everything i can think of for what might be doing it
e.g. a firefox window with flash (video), wmplayer, etc

does anyone have an idea to better debug this?
is there another way to find-out what is keeping it from working?
is there a better power policy program (other than the default one in windows, that for whatever reason, seems to switch between power profiles of its own free will (which i lazily fixed by just setting both of the 2 it switches from having the same information/settings)

My screen is set to shut off at 10 minutes, and I've also noticed that it doesn't always work. I never really thought much about it until reading this though. I'd be interested in seeing if there's a solution.

Do you have an optical mouse on some sort of reflective surface? Sometimes mouse pointers start moving by themselves because of the surface they're on. Could be a specific point on the desk / mouse position.

It's a stretch, but happened to me.
